just spit balling ideas here but if you have a ps2 keyboard and mouse try those and remove all usb hid devices see if you getting any conflicts, also in calibrate i think there is a reset to defaults you could try that.

I have it connected to a laptop with no usb devices connected and it is still doing it. Also, the reset to defaults didn't help.

On another note, there is a settings.ini file that is extracted with the executable. Towards the bottom of the file are the 360 settings. I don't know what these values mean, but they might hold the key if someone can decipher them.

Btw let me understand your issue a bit better, its that the mouse when you move it up goes up right or up left?

Have you turned debug on in the settings.ini it shows some interesting info when you turn on.

Yes, it only happens when the mouse moved straight up and straight down. And yes, it goes up right, then up left, then up right, etc. It almost seems like the X axis is too sensitive or something. And as you increase the sensitivity in MW2, it is more dramatic of a left and right motion.

No, I haven't turned on the debug thing yet. Does it just dump a file into the folder the exe is in? Also, what kind of interesting info is it showing?

Are you seeing the same problem?

found a VERY effective but unelegent solution, its called ANGLE SNAPPING

feature i found in the mouse options on my g500, detects if your trying to move the mouse in a straight line and snaps the mouse to the line.
